Case Management
Case management is another core service of redpoint. We serve as the link between the professionals on your team, keeping them actively informed and involved. Clinical care coordination, engagement, and transitions make up “the work” that is part of living an independent life. We are also here in times of crisis – whether health related or situational – to support you and provide resources. Our purpose is to support you in building your independent life.

Transitions
The best laid plans are built to evolve and grow as your life unfolds. For young adults, this can include moving onto campus or out of the family home for the first time. For others, this may be a realignment of service providers, or transitioning to or from a rehab facility. If you’re moving to Washington, we can set up your new life here, including home scouting, provider referrals, introductions, and recommendations on what the city has to offer that will best meet your life management and social needs.
Crisis Support
A health crisis can be medical or psychological in nature, or a combination of both. We work with people needing support with first or recurring mental health relapses, addiction, or emotional trauma. A family or housing crisis can be an eviction or other imminent loss of a home, or any situation that overwhelms the client or family. We can step in at any time during a crisis.

Medical Advocacy
Clinical care (including doctors, nurses, mental health therapists and more) is often an important part of our client’s journey, but the system and processes to access and manage care can be complex, confusing, and a bit daunting. Our objective is to apply our collective wisdom, resources and best practices as your compass. We can guide you through the system, bringing in professionals as needed to join us as we pursue immediate goals, and to collaborate with us on strategies for future care.
